In Home Lactation Appointment Confirmation

Hi there! Bianca here. I look forward to seeing you at your lactation appointment. Here are a few steps for you to complete so that we are both best prepared for your appointment.

Step 1:  Please read over my consent form/payment policy and privacy policy.

Step 2: Please complete the "New Client Health History Form" linked below.

New Client Health History Form

Now a little information for you.

What to expect at the appointment?
Every appointment is unique depending on what is going on with you and your baby. An appointment typically lasts somewhere around 1.5 hours. Please plan for the baby to be hungry during the appointment. I know that getting any baby to feed at a certain time is not easy, but I will need to see a feeding while I am with you. You can feed the baby as soon as I get there or we can start the feeding a little later if the baby is not quite ready.

If you are pumping or bottle feeding at all, I will want to see your pump and feeding system.  Please have it available.

I will be asking you a lot of questions so that I get a full understanding of you and your baby’s health history. You might be surprised how many different things can impact breastfeeding.  Please share openly anything related to your health, even things before pregnancy.

I may do a physical assessment of your baby and your breasts as it relates to breastfeeding. This may include me touching your breasts. If that makes you at all uncomfortable, please tell me so that I can be particularly respectful of your space.  I may do an oral assessment of your baby which includes me putting my fingers in your baby's mouth and taking some photos.

I will provide you with information and specific suggestions to help resolve any challenges you are facing and help you reach your breastfeeding goals.
After the appointment, I will provide you with a written report that includes my notes, care plan, and additional resources.

I will be sending your OB or midwife and pediatrician a brief summary of our appointment, with your permission.

Because you reached out to me, I know that breastfeeding is important to you.  Facing challenges can be stressful and emotional.  My goal is to be empathetic and understanding while providing current information and effective strategies. I support parents in providing breast milk to their child no matter who they are or how they do that.  I aim to be respectful and mindful of various beliefs, cultures, and family dynamics. Whoever you are, I am here for you.  If you have any special circumstances or requests, please don’t hesitate to discuss them with me.
I look forward to getting to know you and your baby.

See you soon!
